Job Overview We are seeking a dedicated and enthusiastic Admissions Coordinator to join our team. This role is pivotal in guiding prospective residents through the admissions process, ensuring they receive the necessary information and support. This role requires to assist the Administrator in managing the admissions process of the Extended Care Center in accordance with current Federal, state, and local standards, guidelines, and regulations that govern our facility. Responsibilities Promote facility services, assisting prospective residents and their families through the admissions process, and ensuring a seamless transition into our facility. Guide potential residents and their families through the admissions process, including assessments, documentation requirements, and scheduling tours. Conduct clinical assessments to ensure appropriate placement based on care needs. Serve as the primary point of contact for inquiries regarding admissions. Build and maintain relationships with local hospitals, physicians, and other referral sources to promote the facility and increase admissions. Prepare and complete all admission documentation in accordance with facility policy. Welcome new residents and families, give proper orientation and answer questions and provide updates. Collaborate with nursing, therapy, dietary, social service and administrative teams to ensure smooth transitions. Maintain accurate records and input resident information into the Electronic Health Record system. Provide updates to leadership on admissions status and bed availability. Ensure compliance with all Federal, state, and local admission regulations and guidelines. Qualifications & Experience LVN certification required. Minimum 2- 3 years' experience Must be knowledgeable of laws, regulations, and guidelines governing long-term care and personnel administration.
