Position Summary Prestige In Home Care is seeking a motivated and experienced Business Development & Community Outreach Manager to lead our growth and referral-development efforts. The position will initially focus on building relationships and generating new business in the Sacramento area. The Business Development & Community Outreach Manager will later expand outreach into San Francisco, the greater Bay Area, and additional California markets. This is a field-based position requiring regular travel to hospitals, rehabilitation facilities, senior communities, government agencies, nonprofit organizations, healthcare offices, and other potential referral partners. The ideal candidate is confident presenting services, developing professional relationships, following up consistently, and converting outreach efforts into qualified referrals and new clients. Key Responsibilities Develop and carry out a business development and community outreach strategy for Sacramento and future California markets. Introduce Prestige In Home Care and its services to hospitals, rehabilitation centers, skilled nursing facilities, assisted living communities, senior centers, medical offices, and community organizations. Build relationships with discharge planners, social workers, case managers, care coordinators, physicians, community leaders, and other referral partners. Identify new referral sources, partnership opportunities, contracts, and community programs. Schedule and conduct in-person meetings, presentations, facility visits, networking events, and informational sessions. Represent Prestige In Home Care at conferences, health fairs, community events, professional meetings, and outreach activities. Maintain consistent communication and follow-up with prospective and existing referral partners. Generate qualified leads and support the process of converting referrals into new clients. Coordinate with the office team to ensure referrals receive timely follow-up. Develop market-specific outreach plans based on local community needs and business opportunities. Track outreach activities, contacts, referrals, follow-ups, and results using the company's CRM or reporting system. Prepare regular reports on outreach activity, lead generation, referral sources, conversions, and market growth. Monitor competitors, referral trends, and opportunities within each service area. Assist with outreach materials, presentations, brochures, email campaigns, and community-facing communications. Maintain a professional and positive representation of Prestige In Home Care at all times. Travel regularly throughout assigned territories and occasionally attend evening or weekend events. Perform other business development and community outreach duties as assigned. Required Qualifications At least 3-5 years of experience in business development, community outreach, sales, marketing, referral development, account management, or a related position. Experience developing professional partnerships and generating new business. Strong communication, presentation, networking, and relationship-building skills. Ability to confidently approach organizations, introduce services, and conduct professional presentations. Strong follow-up, organization, and time-management skills. Ability to work independently while remaining connected to the office and leadership team. Ability to manage multiple contacts, meetings, territories, and follow-up activities. Comfortable using email, Microsoft Office or Google Workspace, CRM systems, spreadsheets, and digital reporting tools. Valid driver's license, reliable transportation, and willingness to travel regularly. Ability to work in the United States for any employer. Preferred Qualifications Experience in home care, healthcare, senior services, disability services, behavioral health, social services, or a related field. Existing relationships with hospitals, healthcare providers, community organizations, senior communities, or government agencies. Experience working with discharge planners, case managers, social workers, or referral coordinators. Experience opening or developing a new geographic market. Experience working with commission-based performance goals. Bilingual communication skills are a plus.
Compensation and Travel Base salary:
$65,000 per year.
Commission:
Additional commission based on qualified referrals, new clients, contracts, or other agreed-upon performance goals. Approved business travel expenses will be covered or reimbursed according to company policy. Covered expenses may include mileage, transportation, lodging, parking, and other approved business-related costs. The complete commission structure and reimbursement policy will be reviewed during the hiring process. Work Environment This is a field-based role involving regular travel, in-person outreach, professional meetings, presentations, and community events. The Business Development & Community Outreach Manager will begin by focusing on Sacramento and will later travel to San Francisco, the Bay Area, and other assigned California markets. The position may require occasional evening or weekend availability for networking events, conferences, or community programs.
