Home care is a handoff business. Almost every family who calls UniCura is coming from somewhere: a hospital discharge, a rehab stay, a hospice conversation, an elder law appointment, a financial planner who noticed Mom is struggling. This role owns those handoffs. You build the relationships that send families to us, you make sure the transition into our care is fast and safe, and you close the loop with the person who referred them so they refer again. UniCura Health is a new, locally owned non-medical home care company opening November 1 at 7000 Hampton Center in Morgantown. You would be one of our first four office hires, reporting to the Executive Director and working directly with the founder. What you will be accountable for Six jobs, each with a clear measure: Build and keep the referral network. Active, two-way relationships with hospital case managers and discharge planners, skilled nursing and rehab teams, hospice and palliative care, senior living and memory care, elder law attorneys, financial planners and trust officers, and physician practices across Mon, Marion and Harrison counties.
Target:
15 referral sources sending at least one referral a quarter by month 6, on a weekly in-person visit cadence you own. Own the handoff. Take the discharge-planner call, confirm a caregiver and start time, coordinate our Home Again 72-hour post-discharge visit with the Care Coordinator, and send the observation report back to the referring nurse.
Target:
same-day response to every referral; report to the referrer within 24 hours of the first visit. Grow the census. Convert referrals into clients and keep the pipeline honest in AxisCare.
Target:
five new private-pay client starts a month by month 4. Open the Care Oversight channel. Sell our monthly care coordination and oversight subscription through attorneys, planners, trust officers and out-of-state families.

WV Medicaid Personal Care and Aged & Disabled Waiver enrollment, the medical necessity (MNER) process for case managers, VA Community Care, CCM/TCM agreements with physician practices, dementia respite under CMS GUIDE, and the Genesis and Vandalia post-acute collaboratives.

one signed practice agreement and Medicaid enrollment filed by month 6. Represent UniCura. Community events, our partner launch reception, BNI and chamber presence, and a monthly one-page scorecard to every referral partner showing referrals, starts and outcomes. A typical week Three days in the field: hospital and rehab case-management offices, SNF discharge planners, hospice teams, physician practices, attorney and planner offices Two days in the office: referral follow-up, AxisCare pipeline, Home Again (hospital-to-home discharge) coordination with the Care Coordinator, partner scorecards, program build work with the Executive Director Weekly pipeline review with the founder What we are looking for Three or more years in one of: hospital or post-acute case management or discharge planning, home health or hospice liaison or business development, senior living admissions, social work, or healthcare account management Existing relationships in the Morgantown, Fairmont and Clarksburg healthcare community. If you already know the case managers at WVU Medicine, Mon Health, Encompass or the local SNFs by name, tell us Working knowledge of WV Medicaid waiver programs (ADW, Personal Care), VA benefits, or long-term care insurance is a strong plus; willingness to learn them is required Comfortable walking into a hospital unit, a law office and a family kitchen in the same day
