This position promotes the agency's home health services, identifies referral opportunities, coordinates with the clinical and intake teams, and helps increase patient admissions while following Medicare, HIPAA, and agency requirements. Key Duties and Responsibilities Promote the agency's home health services within the assigned territory. Visit hospitals, physicians' offices, clinics, rehabilitation centers, skilled nursing facilities, and senior communities. Build and maintain professional relationships with physicians, discharge planners, case managers, social workers, and referral coordinators. Schedule and conduct presentations, in-service meetings, and community outreach activities. Identify potential referral sources and follow up regularly. Educate healthcare providers about the agency's services, accepted insurance plans, service areas, and referral process. Coordinate referrals with the intake, clinical, and administrative teams. Verify that referral information and required documents are complete and submitted promptly. Monitor referral activity, admissions, conversion rates, and marketing goals. Maintain accurate records of visits, calls, contacts, referrals, and follow-up activities. Prepare weekly or monthly marketing and referral reports for management. Participate in networking events, health fairs, and community programs. Gather feedback from referral sources and help resolve service concerns. Monitor competitors and identify new opportunities for growth. Protect patient confidentiality and comply with HIPAA, Medicare regulations, anti-kickback laws, and agency policies. Avoid making promises regarding admission until the clinical team confirms the patient's eligibility and the agency's ability to provide services. Bachelor's degree in marketing, business, healthcare administration, nursing, or a related field. Established professional relationships with healthcare providers and referral sources in the assigned service area. Familiarity with Medicare, Medi-Cal, managed-care plans, and other insurance networks. Bilingual communication skills are an advantage.
