Provided for approved client travel Job Summary SRK Care at Home is seeking a Maryland-licensed Registered Nurse (RN) for a flexible part-time position performing in-home assessments and supervisory visits for clients receiving home care services. The RN will conduct Initial RN Assessment Visits, develop and review Plans of Care, provide client-specific caregiver training, evaluate caregiver competency, and perform ongoing supervisory visits. Many of our clients receive Maryland Medicaid-funded home care services, so experience with Maryland Medicaid, Residential Service Agencies (RSA), CFC, CPAS, or similar community-based programs is strongly preferred. Responsibilities Conduct Initial RN Assessment Visits for new clients. Perform client reassessments and supervisory visits. Assess personal care, activities of daily living, mobility, safety, and home-care needs. Develop, review, and update individualized Plans of Care. Provide client-specific caregiver orientation and training. Evaluate caregiver skills and competency. Train caregivers on personal care, transfers, mobility assistance, fall prevention, Hoyer lift use when applicable, and other client-specific needs. Monitor the quality and safety of care provided in the home. Communicate with clients, families, caregivers, and SRK Care at Home management regarding changes or concerns. Complete required assessment, care plan, training, and supervisory visit documentation. Follow applicable Maryland RSA requirements and agency policies. Qualifications Active Maryland Registered Nurse (RN) license - Required Home care, community health, senior care, or similar nursing experience preferred. Maryland RSA, Medicaid, CFC, or CPAS experience is a plus. Experience conducting in-home assessments and supervisory visits preferred. Experience developing Plans of Care preferred. Experience training or supervising caregivers/CNAs preferred. Strong assessment, communication, and documentation skills. Valid driver's license and reliable transportation. Ability to travel to client homes. Compensation & Benefits Competitive pay based on experience and visit responsibilities. Mileage reimbursement for approved travel between client locations.
