Independently implements the nursing process for patients and families Provide evidence-based practice resources into clinical practice. Is competent and safe in the operation of Home Health patient care & technology Visits patients according to required visit schedule in order to assess and plan the patient's care. Supervises company personnel assigned to care for the patient. Ensures that qualified caregivers are assigned to care for each patient in accordance with the physician's orders and the nursing assessment. Acts as the liaison between the branch office and the patient, family and source of payment for care. Ensures home environment is safe. Submit clinical and supervisory forms to the branch according to submission requirements. Orient/conduct on-going continuing education to all staff. EXPERIENCE

Bachelor's degree from four-year college or university or previous home care experience preferred; at least three years experience, preferably in a supervisory setting; or equivalent combination of education and experience.
