This is a field-based position responsible for conducting supervisory home visits, client assessments, caregiver oversight, and quality assurance activities throughout our service area. Responsibilities Conduct supervisory home visits for clients receiving personal care and homemaker services. Complete client assessments and reassessments as required. Evaluate client needs and recommend updates to the plan of care. Observe and supervise Nursing Assistants and Homemakers during client visits. Ensure caregivers are providing safe, high-quality care according to agency policies. Monitor client health, safety, and overall well-being. Identify changes in client condition and communicate concerns to the Director of Nursing. Complete accurate and timely clinical documentation. Participate in quality assurance and compliance activities. Provide caregiver education and coaching during supervisory visits. Collaborate with office staff, physicians, hospitals, and community providers to ensure continuity care. Respond professionally to client and family concerns. Qualifications Current, active Rhode Island Registered Nurse(RN) license in good standing. Graduate of an accredited school of nursing. Minimum of two (2) years of nursing experience required. Home care, community health, case management, or geriatric experience preferred. Strong assessment, communication, and documentation skills. Current CPR certification.
