Manage adult patient panels by delivering personalized chronic, acute, and preventive care. Provide comprehensive primary care, including health screenings, vaccinations, and wellness education. Utilize data to support value-based care, identify care gaps, and improve outcomes. Collaborate with interdisciplinary teams for coordinated, high-quality care. Maintain accurate electronic health records and drive practice innovation. Qualifications include MD/DO, active US license, and experience in adult primary care or related fields. Strong communication skills and interest in practice transformation preferred.
