Job Responsibilities:
Working with our team, a
Hospice Care Coordinator:
Develops business by building relationships with referral sources in the community. Identifies and develops ongoing referral relationships with targeted physicians, assisted living facilities, skilled nursing facilities, and hospitals. Community educator on all things hospice. Works with administrator and clinical team to create and develop existing and future business development goals and plans. Meets with patients and families regarding services and care.
