Lowell, MA Job Details Part-time | Full-time $52,757.55 - $63,535.98 a year 1 day ago Benefits 401(k) Paid time off Qualifications Spanish Teamwork Multilingual Computer literacy High school diploma or GED Organizational skills Productivity software Full Job Description Community Liaison (Adult Foster Care) Field-Based Community Outreach & Enrollment Compass AFC is hiring a Community Liaison to expand access to our Adult Foster Care (AFC) programs by building strong referral partnerships and connecting eligible individuals to services. This is a great fit for someone who's energetic, relationship-driven, and enjoys being in the community—meeting people, hosting outreach, and helping families navigate next steps. What you'll do Build referral partnerships Develop relationships with hospitals, rehab facilities, discharge planners, social workers, community agencies, community health centers, and housing organizations. Serve as a consistent point of contact—educating partners on Compass AFC programs, eligibility, and referral procedures. Drive outreach & enrollment Attend and host community events, fairs, and presentations to increase awareness Meet with prospective members and families, clearly explain benefits, and support enrollment in collaboration with our Intake team. Conduct on-site visits with prospective and current clients as needed. Plan, track, and grow Create weekly/monthly outreach plans Track your outreach activity and results (leads, referrals, enrollments) and share insights on what's working. Participate in status meetings and partner closely with Business Development and Intake teams. Support community needs Assist families with basic guidance through government assistance processes (when applicable). Support local caregiver recruitment efforts through outreach and community connections. Provide translation support when needed (bilingual candidates strongly valued). What we're looking for Required 1+ year experience in outreach, admissions, community engagement, or a relationship-based role Comfortable working independently in the field with strong follow-through and organization Strong communication skills and professionalism with community partners Basic tech skills (Google Workspace and/or Microsoft Office) Willingness to work occasional evenings/weekends for community events Preferred / Strong plus Healthcare, home care, social services, insurance, case management, or AFC experience Existing relationships with local community organizations in Lynn and/or Lowell Bilingual or multilingual (a plus): Haitian Creole, Spanish, Khmer Education High School Diploma required An Associate's or Bachelor's degree is a plus What success looks like You build a consistent referral pipeline within your first 60-90 days You reliably meet monthly qualified referral and enrollment goals Partners trust you as a responsive, knowledgeable resource Why join Compass AFC Compass AFC is mission-driven: we help seniors and adults with disabilities receive member-centered care so they can remain safe at home and connected to their communities.
