Overview The Self Direction position is designed for individuals who are passionate about empowering others to take control of their own lives and decisions. This role focuses on supporting individuals in managing their own care and services, fostering independence, and promoting personal growth. The ideal candidate will be dedicated to enhancing the quality of life for those they serve through personalized support and guidance. Duties Assist individuals in developing and implementing their self-directed plans, ensuring that their needs and preferences are prioritized. Facilitate communication between individuals and service providers to ensure that all parties are informed and engaged. Provide training and resources to help individuals understand their rights, responsibilities, and available options within the self-direction framework. Monitor progress towards goals, adjusting plans as necessary to ensure continued alignment with individual aspirations. Advocate for individuals' needs within the community and assist them in accessing necessary services and supports. Provide assistance Mondays or Tuesday for evening activities and events. Provide occasional support on the weekends for weekend events. Must have adequate transportation Must be able to lift a walker. Experience Previous experience in social work, counseling, or a related field is preferred but not required. Strong interpersonal skills with the ability to build trust and rapport with diverse populations. Knowledge of self-direction principles and practices is a plus. Excellent organizational skills with attention to detail to manage multiple cases effectively. Ability to work independently while also being a collaborative team member.
