Job Description

Health Homes Case Manager Harmonia Collaborative Care You are required to go into the community and client homes for this position. A valid NYS driver's license and reliable transportation are required. At Harmonia Collaborative Care, our Health Homes Case Managers help individuals navigate complex systems with consistency, compassion, and follow-through. This role is ideal for someone who values structure, clear expectations, and meaningful work without burnout. What You'll Do
  • Coordinate care for individuals enrolled in the Health Homes program
  • Develop and maintain person-centered care plans
  • Build strong, consistent relationships with members and service providers
  • Connect individuals with medical, behavioral health, and community resources
  • Complete timely and accurate documentation
