Social Services Designee Seymour, WI (15-20 minutes from Green Bay and Appleton)
Starting Salary :
$55,000/year + (Based on Experience) Hours 80 Hours per 2-Week Pay Period | Salaried Position Compassionate care. Connected team. Shared purpose. Position Summary The Social Worker Designee plans, organizes, develops, and directs the Social Services Department in accordance with federal, state, and local standards and facility policies.

This role ensures residents' medically related emotional and social needs are met and maintained on an individual basis. The position oversees and coordinates all aspects of resident admissions, transfers, and discharges while maintaining a strong customer service focus for residents, families, and team members. Responsibilities Participate in completion of the Minimum Data Set (MDS) with the interdisciplinary team Develop, implement, review, and revise individualized care plans with resident and family input Coordinate admissions, transfers, and discharges in compliance with regulations Facilitate complaint and investigation processes Complete required social service documentation, including assessments, care plans, quarterly notes, MDS interviews, Level 1 screens, discharge planning, and related documentation Support residents and families through care planning and transitions
Qualifications Preferred:
