The role combines direct counseling, program oversight, and staff supervision . Key ResponsibilitiesParticipant Support & Case Management Develop, implement, and monitor Individual Service Plans (ISPs) Provide individual and group counseling and conflict resolution Track participant progress and outcomes Maintain accurate, compliant case documentation Prepare reports for families, case managers, and agencies Coordination & Communication Act as a liaison between participants, families, care teams, and external agencies Participate in interdisciplinary meetings and program evaluations Share regular updates on participant progress Supervisory & Program Oversight Supervise counseling staff (schedules, documentation, duties) Manage intake, referrals, enrollment, and discharge processes Ensure compliance with regulatory and organizational standards Train and support staff on counseling practices Coordinate with nursing staff on care plans Additional Duties Assist participants with transportation and mobility Support other departments as needed Perform administrative tasks (email, systems like StoriiCare) Contribute to an inclusive, respectful program environment Qualifications Master's degree + 2 years relevant experience
