Temp-to-Hire What You'll Do Conduct in-depth assessments over the phone with individuals and families to understand their housing, financial, health, and employment needs Help members develop and follow personalized housing plans designed to overcome barriers and achieve stable housing Provide culturally sensitive, one-on-one case management tailored to each member's individual needs Support clients with life skills, job readiness, budgeting, and other tools that promote long-term self-sufficiency Serve as a liaison between tenants and landlords to help resolve concerns and prevent evictions Conduct home visits after housing placement to support continued stability Coordinate with service providers and participate in team meetings to ensure connected, comprehensive care Maintain accurate and detailed documentation and provide weekly and monthly updates to supervisors Support housing transition and sustainability services, nursing facility-to-home transitions, post-hospital housing, and housing navigation Conduct extensive phone outreach to members, community partners, and service providers to coordinate care and maintain consistent communication Perform in-person client visits once training is completed What You Need Education & Experience High school diploma or GED required At least 1 year of experience in case management, healthcare, social services, customer service, or a related field Skills & Qualifications Valid California driver's license and current auto insurance required; mileage is reimbursed Strong communication, organization, and active listening skills Comfortable using computers for documentation and communication Ability to work independently while also contributing as part of a team Ability to multitask and effectively manage multiple clients and priorities Compassionate, patient, and comfortable supporting individuals through challenging situations Strong problem-solving skills and the ability to remain calm under pressure Bilingual English/Spanish skills are a strong plus Ability to sit for extended periods and lift up to 15 lbs.
