Hope Services is seeking a compassionate and dedicated Case Management Specialist to join our team in the Bay Area, providing unparalleled support and services to individuals with developmental disabilities. In this key role, you will be responsible for assessing client needs, planning and coordinating support services, and ensuring the optimal delivery of therapies and training programs. This position not only requires a deep understanding of disability services but also a heartfelt commitment to improving lives within our community. Responsibilities Assess individual client needs through consultations with clients, families, and other professionals Develop, implement, and review personalized care plans in collaboration with a multidisciplinary team Coordinate and monitor services, including therapy sessions, job training, and social activities, ensuring align with client goals Maintain detailed records of client progress and service efficacy Liaise between clients, families, and service providers to promote effective communication and service delivery Advocate for client rights and services, addressing any issues or barriers they face Participate in training and professional development activities to stay informed on best practices in disability services Required Skills Strong interpersonal and communication skills Effective case management and organizational abilities In-depth understanding of developmental disabilities Ability to handle sensitive information with discretion and empathy Proficiency in managing multiple tasks and timelines effectively Experience with case management software and Microsoft Office Suite
