- Provide direct hands-on support to clients serviced in the program
- Identify the individuals interest in order to facilitate a job/career of choice
- Identify individuals skills and talents in order to facilitate a job/career of choice
- Identify job/career opportunities for individuals in the community
- Advocate for individuals served in the program
- Complete daily data collection techniques of documentation to ensure individuals needs are being met
- Provide behavior modification strategies as needed
- Conduct initial and ongoing clients skills assessments
- Develop a career and life plan for clients by outlining measurable goals
- Coach clients to increase career aptitude and maintain their employment
- Provide direct support and training to individuals on various job sites
- Assist clients with developing the necessary skills to meet their goals
- Develop and maintain employer contacts to assist clients with obtaining jobs
- Collaborate with clients, families and business to build professional relationships
- Regularly travel to various sites to meet with clients
- Accurately document client information and progress
- Follow company policies and procedures
- Provide other services as needed

- Self-motivated /able to work in isolation with Individuals served General Requirements;
- High school diploma or equivalent required
- 1+ years of experience working with disabled adults required
- 1+ years of experience coaching and mentoring others preferred
- Valid CPR certification required
- Valid driver's license required/Updated Vehicle Registration/Updated Vehicle Insurance
- Bilingual in Spanish and English preferred not required
- Must have reliable transportation
- Ability to multitask, prioritize and manage time effectively
- Excellent verbal and written communications skills
- Self-motivated and comfortable working independently
- Extremely detail-oriented, goal-driven and organized
