Job Description

Job Summary:
Under the supervision of the Human Resource Department, the Supported Living Specialist is responsible for helping adults with intellectual/developmental disabilities connect with their community while also increasing independent living skills. The Supported Living Specialist will assist in the following areas: Job coaching - identifying both job and career advancement opportunities for individuals based on their needs, objectives, and skill sets. Assisting to identify and enhance job skills, suggest improvements to resumes, and ensure that individuals are adequately prepared for job interviews. Supported Community Connections (SCC) - supports the abilities and skills necessary to enable the individual to access typical activities and functions of community life such as those chosen by the general population, including community education or training, retirement, and volunteer activities. SCC provides a wide variety of opportunities to facilitate and build relationships and natural supports in the community, while utilizing the community as a learning environment to provide services and supports. SCC may be provided in a group setting or may be provided on a one-to-one basis. Supported Living - assisting individuals in their home and community to learn and develop independent living skills (activities of daily living, medication management, money management, etc.) through reminders, prompts, encouragement, and modeling. The frequency and type of assistance will vary and be dependent on the individual's needs. Mentorship - promoting self-advocacy through instructing, providing experiences, modeling, and advising. Mentorship also includes assistance in interviewing potential providers and understanding complicated health and safety issues.
Location of Work:
TCO Administrative Department, and the Community Essential Job functions are but not limited to:
  • Identify jobs pertaining to the interest and skill level of people.
  • Identify community resources and activities pertaining to the interest and skill level of people.
Develop and implement effective coaching strategies to help individuals overcome employment-related challenges. Identify individuals' strengths and provide feedback on how to use those strengths to secure employment and/or excel in their chosen career path. Provide information on suitable career paths based on the individuals' skills, qualifications, interests, and experience. Review individuals' resumes and suggesting improvements as needed. Maintaining accurate records of individuals' progress through Therap. Liaising with employers to identify possible job opportunities for Individuals with disabilities. Teaching Individuals with disabilities how to use job-related tools, instruments, and equipment correctly.
  • Provide Transportation to Individuals to and from work until suitable transportation can be identified.
  • Promote Community Integration by ensuring the same activities and venues are utilized as those used by the public.
  • Foster connections between people we support and the community at large to improve social skills.
  • Implement programs and activities designed to meet the individualized needs. Coordinate daily schedules and activities to maximize individual participation. Coordinate and implement programs outlined for assigned individuals. Maintaining supervision levels to ensure health and safety.
  • Provide supports to maintain and/or improve independence.
  • Provides care related to feeding, personal care, behavioral supports, medication administration, etc.
Maintain an environment that is stimulating and conducive to promoting Independence. Executes and maintains comprehensive documentation for people in services including but is not limited to; ISSP's and BISSP's tracking's, progress notes, incident reports, daily activities, attendance, and other necessary agency documentation. Communicate with program management frequently to share information and resolve issues. Work with other staff to share ideas, seek assistance. Ensure proper storage and care of equipment and supplies. Act as an advocate for assigned individuals by ensuring communication regarding the care of people with the Thrive staff, Host Home Providers, and or guardians and families. Provide transportation and monitoring of individuals. Quarterly host home visits to monitor services, review home books and complete medication monitoring Monthly review of all appointments, notes, and other documentation
  • Implement and monitor Individual Support Plans (ISPs)
  • Ensure all services and supports are being provided to individuals Operate vehicle within the rules and regulations outlined by company policy and Department of Transportation guidelines; conduct vehicle inspections daily, complete necessary documents as vehicle inspection form, mileage and air mileage logs, other transportation related agency documentations.
Promote person centered approach and support by empowering each person's needs, desires, and choices. Implement and promote group and individual job coaching. Seek and promote volunteer opportunities. Prevent and report Abuse, Neglect, Mistreatment, Exploitation promptly. Performs other duties assigned.
State Licensing and Certifications:
State Mandated Training:
CPR/First Aide, Positive Behavior Supports, Universal Precautions, Abuse Prevention and Individual Rights, Defensive Driving, Medication administration, and Safety Care. Trainings and Competency evaluations are offered upon hire and annually/quarterly thereafter.
Minimum Qualifications:
Education and/or
Experience:
  • Must be 18 years of age, or older. Must be knowledgeable in Microsoft office and capable of using electronic charting on Therap. Must be able to utilize technology including but not limited to a computer and software applications.
  • Responsible for keeping current training up-to-date and on changes to technology that are essential to completing essential functions of the job
Certification, Licenses or Registrations:
Must have a valid driver's license and maintain good driving record. Upon hire, Community Support Staff shall successfully complete Thrive Community Options required training mandated Federal/State regulations.
Physical Demands:
Must be able to lift a minimum of 50 lbs. Ability to stand longer periods of time, bending and lifting.
