Full-Time Position Overview:
The Success Coach is a vital part of Genesis Hopeful Haven's mission to empower youth in foster care and those aging out of the system. This role blends case management, life skills coaching, administrative tracking, and program support to ensure each participant has the tools, opportunities, and encouragement to thrive. You'll serve as a consistent, supportive presence—helping youth navigate challenges, build resilience, and prepare for independent living.
Key Responsibilities Direct Client Support:
Conduct client intake assessments and develop individualized service plans. Provide at least one hour per week of one-on-one coaching for each client. Assist clients with college applications, ACT/SAT prep, and other educational goals. Help clients obtain legal documents, driver's education, obtain employment and open bank accounts. Monitor and document academic grades weekly and track school attendance. Support youth with housing stability, including home visits and mediations with foster parents when needed. Prepare "Butterfly" participants for a successful transition to independence by completing competency checklists and tracking progress toward goals. Refer clients to community partners and connect them to resources for health, legal, educational, and employment needs. Driving Client as needed to appointments Engage Client within our 8 program pillars (Employment Readiness, Financial Literacy, Education, Civic Engagement, Mental Health, Daily Habits, Housing, Healthy Relationship)
Program & Event Participation:
Plan and facilitate life skills workshops. Assist with program planning for camps, after-school programs, career exploration, holiday events and vocational training. Attend family bonding field trips, weekend activities, and community engagement events. Participate in and help coordinate enrichment activities such as career site visits and cultural outings.
Administrative & Data Management:
Document all services and progress notes in Trust Central in a weekly manner. Complete case notes, service goals, and CANS assessments. Track referrals, program participation, and goal completion for each client and upload into Trust Central. Coordinating Transportation for client events or programs Take photos to document milestones and achievements for newsletters, social media, client files and reports. Maintain organized client files, including required documentation (e.g., birth certificates, IDs, medical records, insurance cards, grades). Generate and submit regular progress reports to the Directors for grant reporting and newsletters. Maintain accurate records on demographics, program impact, and participant satisfaction. Recruit program community partners to teach life skills and vocational training
Community & Relationship Building:
Build strong relationships with foster parents, caseworkers, GAL, and etc. Strengthen connections with community partners, caseworkers, and agencies to increase referrals and collaborative opportunities. Participate in recruitment efforts for enrolling programming participants
Minimum Qualifications:
Bachelor's Degree in Social Work or a related field from an accredited university. Active designation as a Certified Child Welfare Case Manager (CWCM) required. Two years of related experience, or an advanced degree in Social Work. Experience working with children, teens, and young adults, particularly in foster care or at-risk populations. Strong administrative and organizational skills; able to track and report on multiple cases simultaneously. Valid driver's license in good standing and ability to transport clients as needed. CPR certification (or ability to obtain within 30 days of hire). Ability to pass a Level 2 background check. Bilingual skills preferred but not required.
