SUPERVISION RECEIVED
Works under the direction and supervision of the Program Director.
RESPONSIBILITIES INCLUDE
Provide assistance to individuals with disabilities in obtaining competitive employment that is consistent with their vocational goal. Ensure the delivery of the Five Core Services of the Independent Living Program including: Information and Referral, Peer Support, Independent Living Skills Training, Advocacy and Transition in a manner compliant with ACL requirements.
MAJOR JOB FUNCTIONS INCLUDE
Ø Contact new referral, complete Assessments and open Consumer Service Records on eligible referrals. Ø Ensure all reporting requirements and program deliverables are completed accurately, consistently, and on time. Ø Discuss vocational goals and interests, and assist in developing an Independent Living Plan (ILP). Ø Conduct job searches and develop jobs for on-the-job training and competitive or supported employment that is consistent with consumer's interests and abilities. Ø Assist consumers in developing or re-establishing appropriate skills, attitudes, personal characteristics, work behaviors, and functional capacities. Ø Provide employability skills classes, in areas such as completing applications, developing resumes and cover letters, and developing interviewing skills. Ø Conduct job analysis, if necessary, to assure an appropriate placement and understand the essential functions of the position. Ø When necessary provide time-limited job coaching and assist in recommending accommodations to assure a successful placement for the consumer. Ø Offer support and vocational counseling related to disability for a 90-day period to address issues surrounding employment. Ø Ensure Consumer Service Records (CSR) and other reporting requirements are maintained by keeping all information current and documented as to reflect the progress of the Independent Living Plan, including consumer notes, goals, etc. Ø
Provide Five Core Services:
Information and Referral, Peer Support, Independent Living Skills Training, Advocacy and Transition, to increase the consumer's independence. Ø Perform other duties, as assigned.
QUALIFICATIONS
Ø Demonstrated ability of good oral and written communication skills. Ø Ability to pass a level 2 background check. Ø Work independently and problem solve with minimal supervision. Ø Network with area employers to improve employment options Ø Work comfortably in fast-paced high-stress conditions Ø Ability to work with staff and consumers on a cross-disability basis. Ø Work with diverse populations Ø Demonstrated ability to build and maintain rapport with others. Ø Valid Driver's License and access to a reliable automobile. Ø Ability to work with staff and consumers on a cross-disability basis. Ø A copy of a Masters or Bachelors Degree from an accredited college or university and written documentation (resume) of one year of successful experience and employment in counseling, job placement, job coaching or public vocational rehabilitation program; OR , a copy of an Associates in Arts Degree from an accredited college or university and written documentation (resume) of two years of successful experience and employment in counseling, job placement, job coaching or public vocational rehabilitation program, may be used as a substitute for the required college education and experience; OR , written documentation (resume) of four years of successful experience and employment in counseling, job placement, job coaching or public vocational rehabilitation program may substitute on a year-for-year basis for the required college education and experience.
