(NCCPA) is required. The medical license must be free of restriction and no litigation or disciplinary actions may be pending against the incumbent. The incumbent must have Basic Life Support (BLS) certification, DEA and State of Idaho Controlled Substance Registration numbers. Bachelor's degree required. At least 3-5 years' experience as a primary care provider is desirable.
