About Kompassionate Care Services LLC Kompassionate Care Services is a leading home and community-based service provider in Indiana, supporting individuals with developmental and intellectual disabilities as well as aged and disabled individuals under the Health and Wellness Waiver, Indiana PathWays for Ageing Waiver, Family Support Waiver and the Community Integration, Habilitation Waiver and Private Pay. We are committed to compassionate, person-centered care that enhances quality of life. Position Summary We are seeking a dedicated and organized Care Coach to oversee and coordinate services for clients across counties in Indiana. This role involves regular client visitation, ongoing monitoring, incident management, and advocacy to ensure that each individual receives quality care, support in alignment with their Individualized Support Plan (ISP), and caregiver training. Key Responsibilities Conduct quarterly in-person visits to clients to assess well-being and service satisfaction. Conduct client's home safety assessments Complete monthly check-ins , which may be virtual or in-person, as needed. Monitor the delivery of services to ensure compliance with client plans and waiver requirements. Document and report incidents , conduct thorough investigations, and assist in resolution and follow-up actions. Maintain accurate and timely documentation of all client interactions, service plans, and reports. Communicate effectively with clients, families, caregivers, and service providers. Ensure clients' rights are respected and supported. Collaborate with the care team to update ISPs and address evolving needs. Travel throughout Indiana as needed to meet client and organizational needs. Train caregivers Qualifications Bachelor's degree in Social Work, Psychology or Human Services or a related field is required. Minimum 1 year of experience in case management, preferably in home and community-based settings. Strong knowledge of Indiana Medicaid Waiver programs is a plus. Excellent communication, documentation, and interpersonal skills. Ability to work independently and manage a flexible travel schedule. Must possess a valid driver's license and reliable transportation. Proficient with Microsoft Office and case documentation systems. Passion for advocating for individuals with disabilities and the aged.
