Essential Responsibilities:

Role modeling professionalism Develop positive relationships with LIFE residents, families, and staff Daily documentation of activities/goals performed Effectively manage LIFE email account and complete required training Intervene verbally with individuals during behavioral incidents Transport residents to and from employment and volunteer sites as needed (mileage reimbursement provided)

Required Skills/Abilities:

Excellent verbal communication, interpersonal, organizational, and time-management skills; strong creative and problem-solving skills; ability to function well in a high-paced environment; proficient with Microsoft Office. Additional duties may include clearing snow and ice from walkways and vehicles.

Preferred:

Experience with individuals with intellectual disabilities or a vulnerable population; background in retail, hospitality, restaurant, or training environments; high school diploma or equivalent, some college or technical coursework preferred.

•7pm Position Summary Community Case Managers provide socialization, instruction, and daily living supports while serving as positive role models to the adults we support. This role includes both home-based and community-based activities , helping individuals build skills, confidence, and independence. $1,000 sign on Bonus (More info during interviews) Essential Responsibilities Primary responsibilities include, but are not limited to: Role model professionalism and appropriate daily living skills Provide transportation for individuals served when required Attend and actively participate in group and community social activities Build and maintain positive relationships with LIFE residents, families, and staff Support the health, safety, and well-being of individuals served Assist with activities of daily living (planning and attending activities, menu planning, shopping, cooking, etc.) Verbally intervene and de-escalate during behavioral incidents as needed Complete required daily case notes documenting goals, activities, and accomplishments Effectively manage a LIFE email account Utilize Microsoft Office Suite or related software Complete all required trainings (in-person and online) Perform additional duties as assigned by a Supervisor or Director, including substituting for other staff as needed Assist with maintaining safe environments when required (e.g., clearing snow/ice from walkways, vehicles, and parking areas) Required Skills & Abilities Excellent verbal and written communication skills Strong interpersonal and organizational skills with attention to detail Effective time management with the ability to meet deadlines Creative thinker with strong problem-solving skills Ability to work in a fast-paced and occasionally stressful environment Positive, patient, and supportive demeanor Ability to work both independently and as part of a team Ability to provide consistent encouragement, interaction, and support to clients Preferred Education & Experience Experience working with individuals with intellectual disabilities or other vulnerable populations preferred (personal experience or Special Olympics involvement valued) Strong computer skills Comfortable working in community-based, people-facing roles (extroverted personality a plus) High school diploma or equivalent required; some college or technical coursework preferred Patient and kind composure is mandatory Physical Requirements & Work Environment Ability to maintain the pace of individuals supported Ability to lift up to 10 pounds Ability to work in varied environments including walking, standing, and sitting Additional Requirements Candidates must be able to successfully complete all LIFE pre-employment screening requirements, including: Professional reference checks CORI (Criminal Offender Record Information) DPPC (Disabled Persons Protection Commission) DDS fingerprint-based criminal background check Motor vehicle driving history acceptable to LIFE's insurance carrier Completion of all required in-person and online trainings Reliable transportation is required.
