Full-Time About the Role We are seeking a compassionate and experienced PACT Addiction Specialist to join our multidisciplinary Program of Assertive Community Treatment (PACT) team. This role provides clinical support, substance use treatment, rehabilitation services, and case management to individuals living with serious mental health conditions and co-occurring substance use disorders. The Addiction Specialist serves as the team's substance use expert, helping individuals develop recovery goals, relapse prevention strategies, and skills needed to achieve greater independence in the community. Key Responsibilities Provide case management and individualized treatment planning for assigned individuals Conduct assessments related to mental health, substance use, daily living skills, and overall functioning Develop and support recovery plans using motivational interviewing and stage-based treatment approaches Provide individual, group, and family support services in office and community settings Assist individuals with symptom management, relapse prevention, crisis intervention, and recovery goals Coordinate care with families, community providers, treatment programs, and other support services Provide support with housing, employment goals, transportation, healthcare access, and activities of daily living Participate in team meetings, treatment planning, documentation, and shared shift management responsibilities Provide occasional on-call crisis support, including evenings/weekends as needed Qualifications Bachelor's degree in a human services-related field preferred 3-5 years of supervised clinical or rehabilitation experience working with individuals with psychiatric disabilities preferred

CADAC, LADAC

LADAC II

certification required Experience working with individuals with co-occurring mental health and substance use disorders Strong communication, assessment, crisis intervention, and advocacy skills Ability to work independently while collaborating as part of a multidisciplinary team Commitment to supporting individuals' recovery, independence, and personal goals Additional Requirements Valid driver's license for 6+ months and reliable transportation required Must maintain a safe driving record and meet agency driving requirements Ability to provide community-based services, including assisting individuals with daily living activities CPR and First Aid certification required within two weeks of hire Additional PACT training and certifications required after hire Why Join Our Team?
