Schedule:
Monday-Friday Full time: 40 Hours to fit the needs of the program.
Pay:
$23 per hour - Paid training provided by Venture!
Sign On Bonus:
$1000 for Full Time and $500 for Part Time employees
SUMMARY OF POSITION
The Individual Skills Caseworker reports to the Director of Day and Community Services and provides assessment and/or teaching of community living skills to developmentally delayed persons that facilitate the individual's ability to remain in current home environment. The Individual Supports Caseworker delivers direct training to individuals in compliance with Agency policy and procedures. This position will be compensated on an hourly basis.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Provide home visit to assess new referrals and implement Individual Skills Support strategies weekly. Maintain daily activity statistics and documentation as required by Agency and contract policy and procedure. Represent the department in multi-disciplinary meetings for planning and review of Individual Support Plans. Participate in all required staff and Agency meetings. Provide direct individual training in community, of community living skills and defined by program contract. Attend workshops and other continued education programs. Report to the Program Director of Shared Living, AFC & Individual Supports for accountability of scheduled hours. Compose reports as required by Agency and contract policy and procedures Maintain confidentiality and positive demeanor while successfully juggling multiple priorities simultaneously. Maintain confidential and budgetary records in accordance with developed funds management protocols specific to each person supported. Communicate with Program Director of Shared Living, AFC & Individual Supports regarding service delivery reports and contract hour goals progress.
MARGINAL DUTIES AND RESPONSIBILITIES
Attendance at administrative and other meetings as required. Provide recommendations to your supervisor regarding ways to improve the current systems Observe all Agency policies and procedures Accept responsibility and accountability as part of the position Maintain confidentiality and positive demeanor while successfully managing multiple priorities simultaneously. Maintain current CPR and First Aid Certifications. Presence in administrative office as needed Other duties as assigned.
PHYSICAL AND MENTAL REQUIREMENTS
Effective verbal and written communication skills Ability to establish a professional rapport with the developmentally disabled population, their families, co-workers and service providers. Possess impeccable time management and organizations skills Ability to make and keep appointments in the community. Ability to analyze tasks for step by step instruction. Ability to report performance to Program Director of Shared Living, AFC & Individual Supports. Strong interpersonal and communication skills, and the ability to work with a wide range of employees. Organized, dependable and even temperament; good phone manner. Working knowledge in Microsoft Office and office equipment. Must be able to bend, lift, stoop, reach and file.
Requirements:
MINIMUM ACCEPTABLE QUALIFICATIONS
Bachelor's Degree in education, psychology, rehabilitation or related field and one year professional experience preferred; OR 60 credits toward a Bachelor's Degree with continuing study and 3 years' experience in special education. Must be 21+ years old, have a High School Diploma or GED, a valid driver's license, daily use of a car on site and be available by telephone for daily contact. Employee must possess the knowledge; skills and abilities listed above, and demonstrate that they can perform the essential functions of the job. Employee must be able to speak, read and write in fluent English.
