Job Description

Foundations Group is seeking a compassionate, organized Rehabilitation Coordinator for our adult Psychiatric Rehabilitation Program in Linthicum. You will manage an assigned caseload and provide individual and group rehabilitation services that help participants practice daily-living, wellness, social, and community-integration skills tied to their Individual Rehabilitation Plans. What you'll do Deliver person-centered individual and group psychiatric rehabilitation services. Help participants practice and generalize skills through explanation, modeling, guided practice, feedback, and community-based learning. Track caseload needs and participant progress; communicate changes and concerns to the PRP team. Complete accurate, individualized service documentation in the approved electronic record. Coordinate with treatment providers, care coordinators, families/supports, and community resources when authorized. Support rights, confidentiality, safety, incident reporting, crisis-response procedures, and discharge/transition planning. Participate in training, supervision, competency review, team meetings, record audits, and quality-improvement activities. What this role is—and is not This position focuses on psychiatric rehabilitation and skill development. It does not independently provide psychotherapy, diagnose conditions, prescribe or change medication, or perform functions reserved to licensed clinicians or the program's qualified Rehabilitation Specialist. What you bring High school diploma or recognized equivalent required; relevant college education is preferred. Relevant behavioral-health, rehabilitation, human-services, or serious-mental-illness experience sufficient to perform assigned duties competently. Clear, respectful communication and strong documentation, organization, teamwork, and computer/EHR skills. Ability to complete required screening, orientation, training, confidentiality requirements, and competency validation. Valid driver's license and acceptable driving record if participant transportation is assigned.
