Job Description

POSITION SUMMARY
The Full-Time Interventionist is a trusted, community-based frontline professional who builds relationships with people at the highest risk of involvement in firearm violence. The Interventionist identifies emerging conflicts, supports peaceful resolution, reduces retaliation risk, connects participants to requested services through warm handoffs, and documents work in accordance with grant, confidentiality, and safety requirements. Effectiveness depends on credibility, consistency, cultural awareness, sound judgment, and the ability to engage people without coercion.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Develop consistent, respectful relationships with individuals, groups, and communities facing the highest likelihood of involvement in firearm violence. Maintain regular communication through approved outreach, meetings, outings, community presence, and follow-up strategies. Identify situations in which conflict or retaliation may occur and use approved de-escalation, mediation, safety-planning, and conflict-resolution practices. Work with participants to identify nonviolent alternatives, personal goals, immediate safety needs, and requested supports. Provide referrals and warm handoffs to behavioral health and substance-use treatment, case management, therapeutic services, education, vocational programs, employment assistance, housing, legal, reentry, and other wraparound resources. Destigmatize access to behavioral health and social services while respecting participant choice, privacy, and readiness. Maintain timely records of engagement attempts, individuals engaged, conflicts identified, conflicts resolved, conflicts with ongoing or unknown outcomes, referrals made, warm handoffs, and service engagement. Use approved referral, triage, communication, documentation, and data-entry tools; contribute accurate information for REDCap and grant reporting without overstating outcomes. Participate in case reviews, supervision, partner coordination meetings, listening sessions, quarterly quality-improvement reviews, and evaluation activities. Complete required training and demonstrate readiness through mock referrals, crisis-response simulations, safety exercises, communication drills, and debriefing. Follow field-safety plans, check-in/check-out procedures, confidentiality standards, incident-response protocols, boundaries, and reassignment directions. Share community observations and participant feedback to strengthen the written CVI plan, referral priorities, operating procedures, and partnership network. Support community engagement and outreach activities as assigned by the Project Director.
