Job Description
Hope Network is seeking a Job Coach for our Cadillac Center to support individuals with disabilities and complex needs in gaining and maintaining employment. In this role, you will provide person-centered vocational coaching, job readiness training, and on-the-job support to help participants build skills, independence, and confidence. You will collaborate with employers, care teams, and families, document progress, and adjust plans based on goals and performance. This is a first-shift, community-based position in a mission-driven, interdisciplinary environment focused on inclusion, dignity, and long-term success at work. Responsibilities Provide on-the-job coaching and support to individuals with disabilities and complex needs. Deliver job readiness and vocational skills training tailored to participant goals. Collaborate with employers to identify suitable job tasks and accommodations. Coordinate with interdisciplinary care teams, families, and community partners. Develop and adjust individualized employment plans based on performance and goals. Document participant progress, outcomes, and service notes accurately and timely. Support participants in developing soft skills such as communication and time management. Advocate for inclusive employment practices with community employers. Assist with job development, placement, and retention strategies. Maintain a safe, respectful, person-centered environment at all times.
