Core Job Expectation:
Accept new clients and complete intakes Complete annual renewals, quarterly visits and semiannual meetings with clients Coordinate interdisciplinary meetings Continuously monitor client files for compliance Help recruit new staff Attend team meetings Handle emergency situations Complete file audits File Maltreatment & internal investigations reports Evaluate program for compliance Train, supervise and support Staff General Skills Excellent communication and interpersonal skills Great patience for individuals suffering from pain and mental health Experience with disability Programs & Services Experience with working case mangers and care coordinators Qualified Designated Coordinator qualifications 1) a baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; (2) an associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; (3) a diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older; or (4) a minimum of 50 hours of education and training related to human services and disabilities; and (5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
Required training after hiring:
Pass the DHS NET study 2.0 criminal background study before providing services Complete Designated Manager/Designated Coordinator training Complete the QP training within six months of hire (Steps for Success)
