Job Description

QUALIFICATIONS
Bachelor's Degree or equivalent experience as outlined in MN 245D license requirements. Valid Driver's License and insurable driving record along with ability to pass Criminal Background Check. Must be able to fluently read, write and speak in English language and communicate effectively and efficiently. Organized, strong work ethic, ability to multi-task and prioritize.
JOB SUMMARY
Responsibilities:
Provides case management, job training, and vocational support to individuals with disabilities receiving services through the Waivered Services Department. Ensures all services meet
DHS 245D
Home and Community-Based requirements for Day Support, Employment Exploration, Employment Development, Employment Support, and Pre-Vocational Services. Supports each person in achieving their employment goals by coordinating services, delivering vocational programming, and building strong relationships with service providers and employers.
PRIMARY DUTIES
Actively participates in developing the Individual Support Plan Addendum and identified outcomes. Provides training, guidance, and ongoing support to Direct Support Professionals to ensure accurate implementation of the support plan addendum and achievement of individual outcomes. Responsible for organizing and conducting Intake, Initial Planning, Semi‑Annual, and Annual meetings for individuals on your caseload. Completes and distributes all required documentation prior to each meeting. Prepares thorough written Progress Reports and completes or reviews the Support Plan Addendum, Individual Abuse Prevention Plan (IAPP), Self‑Management Assessment (SMA), and all other required 245D documentation. Completes and submits data necessary in Vertex daily to meet billing and payroll deadlines. Provides job coaching to individuals on your caseload and offers training and guidance to other Job Coaches. Delivers one‑to‑one instruction and task training for persons served in in‑house, crew‑based, or community employment settings. Ensures all training is tailored to each individual's needs, using appropriate instructional strategies such as hand‑over‑hand support, verbal cues, positive reinforcement, and other person‑centered approaches. Completes Department of Human Services (DHS) RMS worksheets to request the units needed to provide services for individuals on your caseload and submits them to the individual's social worker. Monitors authorized units and tracks usage to ensure services remain within approved limits. Completes required monthly case notes that share relevant, specific details on face- to- face contacts a minimum two times per month.
SALARY AND BENEFITS
Starting salary: $26.50 per hour. WCI offers competitive salary and benefits packages.
