The QP promotes person-centered, safe, effective, and properly documented services consistent with each individual's assessed needs, service plan, and applicable North Carolina requirements. North Carolina Qualification Requirements The individual designated as a QP must meet an applicable North Carolina qualification pathway and must have the knowledge, skills, abilities, education, and qualifying experience appropriate to the population served. Qualification documentation must be verified and maintained in the personnel record before QP designation. A master's degree in a human services field with the qualifying experience required for the population served; A bachelor's degree in a human services field with the qualifying experience required for the population served; A bachelor's degree in a field other than human services with the additional qualifying experience required by North Carolina rules; An applicable professional license, provisional license, certificate, registration, or permit when qualifying through a professional credential pathway; or An applicable associate degree in a human services field from a community college plus at least two years of qualifying supervised

experience with the population served, when all current North Carolina requirements for that pathway are met. Required Experience and Competency Documented experience working with the specific population served by I Am Hope Agency LLC. Experience with person-centered planning, goal implementation, progress monitoring, service coordination, and documentation review. Ability to recognize and respond appropriately to health, safety, behavioral, rights, and service-delivery concerns. Experience providing guidance, training, supervision, or professional oversight to direct-support personnel, when required by the service. Knowledge of confidentiality, incident reporting, abuse/neglect/exploitation reporting, individual rights, community integration, and quality improvement. Essential Duties and Responsibilities 1. Provide professional oversight for assigned individuals, staff, and services. 2. Review assessments, person-centered plans, goals, strategies, authorizations, and service documentation. 3. Monitor progress toward individual goals and identify changes in needs, risks, or supports. 4. Complete required supervisory contacts, observations, reviews, and follow-up activities. 5. Guide and support direct-care staff in implementing individualized services correctly and consistent. 6. Review progress notes and related records for accuracy, completeness, timeliness, and consistency with the service plan. 7. Participate in treatment and support-team meetings and coordinate with families, guardians, care managers, healthcare professionals, and other authorized providers. 8. Ensure individuals are treated with dignity, respect, privacy, choice, and person-centered consideration. 9. Identify service deficiencies and assist with corrective action, staff retraining, and quality-improvement activities. 10. Follow agency procedures and applicable requirements for incidents, emergencies, confidentiality, documentation, health and safety, and mandatory reporting. 11. Maintain professional competency and complete all required orientation, annual, population-specific, and service-specific training. Personnel File Verification Before an employee is assigned QP responsibilities, I Am Hope Agency LLC will verify and maintain documentation supporting the individual's qualifications. Documentation may include a resume/application, verified educational credentials, applicable professional license or certification, employment verification, records of supervised qualifying experience, training records, and other documentation demonstrating competency with the population served.
Pay:
$22.00 - $25.00 per hour Expected hours: 40.0 per week
