Qualified Professional

This posting is for a full time Qualified Professional. The Qualified Professional will maintain a home-like environment and participates daily in the direct services of the group home. Qualified Professional will maintain confidential records on all individuals and families, which include individual goal planning, progress toward goals, medical information, and assessments of resident adjustment (HRST). Qualified Professional will help to provide opportunities for individuals to participate in goals as indicated in their PCP, including scheduling and transportation to and from places of employment, personal appointments, and for recreational and other events. Ensures that support professionals are actively engaged with clients during all activities as appropriate, supporting individuals in learning and being as independent as possible. Other responsibilities of a Qualified Professional may include: Developing and monitoring person-centered plans Planning and engaging in activities Providing counseling and crisis management Helping families meet basic needs Entering service notes and other documentation Providing guidance for professional development Qualified Professionals should also have good communication skills and be able to read, analyze, and interpret information. The Qualified Professional will also work closely with the office staff. This individual must also have experience using ShareNote system.
