Position Summary:

We are seeking a Primary Case Coordinator (PCC) to provide services to adults enrolled in our mental health program. The PCC plays a key role in supporting individuals through structured therapeutic programming and individualized case management to help normalize lifestyles and foster community integration.

Key Responsibilities:

Facilitate a variety of therapeutic and psychoeducational groups for program participants. Manage a caseload of clients, providing individualized support, guidance, and follow-up. Develop, implement, and monitor individualized service plans in alignment with each client's goals. Assist clients in accessing community resources and integrating into daily community life. Collaborate with interdisciplinary team members to ensure coordinated and comprehensive care. Maintain accurate, timely, and professional documentation of services, progress notes, and treatment plans in accordance with agency and regulatory standards. Participate in team meetings, supervision, and ongoing training to support professional growth

Required Qualifications:

Bachelor's Degree in Social Work, Psychology, Rehabilitation, or a related Human Services field (required). Demonstrated ability to work effectively with individuals experiencing mental health challenges. Excellent verbal and written communication skills. Strong documentation and record-keeping skills with attention to detail and compliance with regulations. Ability to build trust and maintain professional boundaries with clients. Competence in time management and prioritizing multiple responsibilities in a fast-paced environment. Proficiency in using computers and electronic health records (EHR) or case management systems. Commitment to upholding confidentiality, ethical standards, and trauma-informed care practices.

Preferred Qualifications:

Two (2) years of experience in mental health case management, rehabilitation, or a related human services field. Experience facilitating therapeutic or psychoeducational groups. Knowledge of local community resources and support systems. Familiarity with Medicaid, insurance documentation, or state reporting requirements. Pay:

$35,000.00 - $37,000.00 per year
