  • $20/hr AHRC Nassau provides support to over 2,200 people throughout Nassau and Suffolk Counties.

In this role, you will: Support the development and ongoing revisions to individual Staff Action Service Plan by providing input and information. Track individual progress towards desired goals and objectives in Life Plan. Segment job duties and assignments into specific steps to facilitate the learning of the people we serve. Monitor individual level of performance, including determining when support can be maintained, increased, or gradually faded. While in assigned community worksites, represent agency in a professional and exemplary manner. Serve as a liaison between people we serve and worksite employees/employers to facilitate appropriate work readiness skills training and professional work relationships. Maintain accurate records on job coaching services provided to each person we serve. Respond to crisis situations in a humane and proactive manner and in accordance to policy procedures and training. Develop and maintain written documentation, as requested.
We require:
