Position Summary:
Unison Health is seeking a Substance Abuse Specialist to join our Assertive Community Treatment (ACT) Program, an evidence-based model of delivering comprehensive community-based behavioral health services to adults with serious and persistent mental illnesses who have not benefited from traditional outpatient treatment. The ACT model utilizes a multidisciplinary team of providers who work closely together to deliver services designed to meet clients' needs, including substance use treatment, vocational support, and coordinated team-based care.
Key Responsibilities & Role Highlights:
Provide services to assist in resolving or minimizing the effects of a client's impairment. Complete assessments, monitoring of conditions, and crisis interventions. Provide education, linking and training in community resources to clients. Assistance in daily living skills and help with obtaining necessary services for meeting basic human needs and entitlements. Participates as a member of the ACT treatment team and serves as a consultant to the team in the area of substance abuse.
Education & Experience Requirements:
QMHS 3+ with high school diploma required. Bachelor's degree preferred. CDCA (can be obtained within 6 months of hire). Experience in integrated dual disorders treatment preferred. Unison Health is an Equal Opportunity Employer.
