Central Outreach is seeking a full-time Mobile Health/Hep C Tester to join our team—recognized as the #1 curer of Hepatitis C in the United States. This role offers the opportunity to make a meaningful impact by supporting patients through their treatment journey while expanding access to care in the community. The Mobile Health/Hep C Tester plays a critical role in delivering community-based healthcare services through the Hepatitis C (Hep C) Program. This position is responsible for conducting mobile testing for Hepatitis C, HIV, and sexually transmitted infections (STIs), facilitating linkage to care, and supporting patients through treatment and follow-up processes. Mobile Testers work in diverse community settings including correctional facilities, rehabilitation centers, addiction treatment programs, incarceration/workforce environments, and with unhoused populations. This role is both clinical and relationship-driven, requiring a self-directed, proactive individual who is comfortable engaging with patients and staff while building sustainable partnerships that expand access to care. This position offers a base salary along with performance-based incentives tied to key outcomes, including the development of new and recurring testing partnerships and the number of Hepatitis C consults successfully facilitated. The role includes monthly goals to guide outreach efforts and support continued program growth. Phlebotomy is a critical component of this role. While prior experience is not required, candidates must be comfortable being trained and performing blood draws independently in the field. Key Responsibilities Community Outreach, Partnership Development & Engagement Establish, develop, and maintain new community partnerships to expand testing access Identify and secure recurring testing sites for ongoing service delivery Actively network within the community to grow program presence Build relationships with: Correctional facilities Rehabilitation and addiction treatment centers Community programs and shelters serving unhoused populations Lead group education sessions to: Normalize testing Reduce stigma Encourage participation Maintain strong, professional relationships to ensure consistent scheduling and program growth Patient Intake, Assessment & Testing Perform rapid testing for: Hepatitis C (HCV) HIV STI screening Obtain and document vital signs: Height, weight, blood pressure, pulse, respiratory rate, etc. Facilitate STI screening, including specimen collection (urine, blood, swabs) Complete all required documentation: Consent forms Face sheets Release of Information (ROI) Accurately assess patient history (HCV, HIV, treatment status) Care Coordination & Linkage to Treatment Coordinate real-time telehealth visits with providers using HIPAA-compliant platforms Communicate patient information to providers during consultations Assist with scheduling follow-up appointments and referrals Support linkage to: Hepatitis C treatment HIV care PrEP services In-clinic services when appropriate Insurance & Case Management Support Verify insurance eligibility at every patient encounter Assist uninsured patients by: Connecting them with Case Managers Facilitating insurance enrollment processes Collect and communicate required documentation for insurance applications Participate in assistance tracking workflows Electronic Medical Record (EMR) Documentation (Epic, SalesForce) Create and manage patient charts in Epic Enter accurate demographic and insurance information Add patients to appropriate resource schedules Document within EMR based on SOP and established processes Upload consent forms and supporting documentation Maintain accurate and timely documentation for all encounters Laboratory Collection & Phlebotomy Perform venipuncture and specimen collection per protocol Label specimens accurately and completely Process and store specimens according to guidelines and deliver specimens to approved specimen drop locations within stability requirements Document all collected and attempted labs Communication & Team Collaboration Communicate with team members via: Secure messaging platforms EMR workflows Notify providers of: Positive test results (especially HIV) Need for consults or lab orders Collaborate with: Nurses Providers Case Managers Administrative staff Patient Engagement & Follow-Up Support Provide patient-centered, non-judgmental care with cultural humility Build trust and rapport to encourage testing and treatment engagement Assist in facilitating patient follow-up and engagement Support RN and provider efforts in contacting patients Reinforce education on: Medication adherence Prevention strategies Follow-up care Territory Coverage & Travel Cover an assigned geographic territory with an approximate 2-hour driving radius Travel daily to outreach locations, facilities, and patient homes using a company vehicle Maintain company-provided vehicle and coordinate with fleet management for maintenance Manage scheduling and logistics to maximize efficiency Maintain a flexible schedule based on facility and patient needs Ability to manage a caseload of up to 30 facilities Home Visits & Field Safety Conduct home visits when appropriate Follow safety protocols for field-based work Ensure proper documentation and check-in procedures Maintain professionalism in all environments General Responsibilities Maintain company-provided equipment Demonstrate punctuality and reliable attendance Maintain a professional attitude and demeanor at all times Adapt to a fast-paced, evolving, and sometimes unstructured work environment Effectively multi-task and prioritize responsibilities Perform additional duties as assigned Compensation Structure Base salary Performance-based incentives, including: Development of new and recurring testing partnerships Number of Hepatitis C consults successfully facilitated Required Qualifications High school diploma or equivalent required Valid driver's license with a clean driving record (minimum 3 years) Willingness to be trained in phlebotomy required Ability to travel daily within assigned territory Preferred Qualifications Experience in community health, outreach, or public health Sales or outreach experience Phlebotomy experience Experience working with underserved populations Required Skills & Competencies Strong interpersonal and relationship-building skills Friendly, outgoing personality with ability to engage patients and staff Ability to make individuals feel comfortable and safe during testing Self-directed, proactive, and Justice-involved individuals Unhoused populations Strong organizational and time management skills Ability to manage multiple workflows simultaneously Basic computer skills (Excel, Word, EMR systems preferred) Physical & Work Environment Daily travel within assigned territory Ability to stand, walk, and perform clinical tasks for extended periods Exposure to clinical environments and biological specimens Flexible schedule based on outreach needs
