Job Description
Overview We are seeking a compassionate and dedicated Job Coach and Job Support Professional to join our team. In this role, you will provide essential support to individuals with varying needs, helping them achieve their career goals and enhance career skills. This position is ideal for those with experience with individuals with mental disabilities and who are passionate about caregiving. Duties Demonstrates knowledge of and complies with all established policies and procedures of the agency, county, and Commonwealth of Pennsylvania. Demonstrates professional, ethical, and positive behavior while representing the agency and supporting individuals in the workplace and community. Provides individualized employment support based on the person's abilities, interests, preferences, goals, and employment plan. Assists individuals with identifying appropriate employment opportunities based on their skills, interests, abilities, and career goals. Assists individuals with completing job applications, preparing resumes, and developing appropriate job-search skills. Provides support during the interview process, including interview preparation, appropriate workplace communication, and professional presentation. Assists individuals in developing and maintaining appropriate workplace behaviors and social skills. Provides individualized on-the-job training and coaching to help the person learn job duties, workplace procedures, and employer expectations. Breaks job tasks into manageable steps and uses appropriate teaching and prompting techniques to promote successful job performance. Gradually fades prompts and direct support as the individual develops independence and demonstrates competency. Supports individuals in learning and following employer policies, procedures, schedules, safety requirements, and workplace expectations. Provides hand-over-hand, physical, verbal, visual, or other appropriate supports when necessary and authorized. Promotes independence and self-advocacy rather than creating unnecessary dependence on staff. Assists individuals with developing appropriate workplace relationships and communication with coworkers, supervisors, customers, and members of the public. Provides support with workplace problem-solving, conflict resolution, and appropriate responses to challenging situations. Supports individuals with maintaining appropriate attendance, punctuality, work schedules, and time-management skills. Assists individuals with transportation training and/or transportation to and from the workplace when required and authorized. Provides community-based employment support and ensures the individual has access to appropriate supports necessary to participate successfully in employment. Maintains professional communication and positive working relationships with employers, coworkers, families, support teams, and service providers. Communicates with employers regarding the individual's training needs and performance while maintaining appropriate confidentiality. Advocates for reasonable workplace accommodations and supports when appropriate and authorized. Assists individuals in learning workplace safety procedures and recognizing potentially unsafe situations. Monitors the individual's progress toward employment goals and identifies areas where additional training or support may be needed. Provides ongoing coaching and reinforcement to help individuals maintain employment and improve job performance. Assists with job retention by identifying barriers to successful employment and working with the individual and support team to develop appropriate solutions. Supports individuals in exploring opportunities for increased hours, additional responsibilities, advancement, or other career-development goals when appropriate. Completes required employment assessments, evaluations, progress notes, and other documentation accurately and within required timeframes. Records and reports the individual's progress toward employment goals and objectives. Completes daily service documentation, job-coaching notes, timesheets, and other required records clearly and accurately. Documents services provided, including the type, duration, location, and purpose of employment support as required. Reports significant changes, concerns, incidents, injuries, or barriers affecting the individual's employment or well-being to the appropriate supervisor or specialist. Follows all applicable incident-reporting requirements and completes required reports within established timeframes. Attends required team meetings, staff meetings, trainings, employment meetings, and planning meetings concerning the individual. Participates in the development, implementation, and review of the individual's employment goals and service plans. Implements the individual's approved service plan, employment plan, and behavioral or support strategies as written. Maintains confidentiality of all personal, medical, employment, and service-related information. Follows HIPAA and other applicable confidentiality and privacy requirements. Maintains professional boundaries with individuals, employers, coworkers, families, and other members of the support team. Provides appropriate feedback to the supervisor, Employment Specialist, Program Specialist, Supports Coordinator, or other designated professional regarding the individual's progress. Reports concerns regarding discrimination, harassment, unsafe working conditions, exploitation, abuse, neglect, or other serious concerns through the appropriate channels. Encourages individuals to exercise choice, control, self-determination, and dignity in employment-related decisions. Promotes inclusion and integration within the workplace and community. Serves as a positive role model for appropriate workplace behavior, communication, professionalism, and problem-solving. Maintains required certifications and completes all agency, county, state, and employment-related training requirements. Performs other duties related to employment support and individualized services as assigned. Requirements Must be at least 18 years of age. Must obtain one of the following within 9 months of hire : Certified Employment Support Professional (CESP) , ACRE-approved Basic Employment Services Certificate of Achievement , or ACRE-approved Professional Certificate of Achievement in Employment Services . Must be a high school graduate or have a GED Plus. Must have a valid driver's license Must have a good driving record. Must have a vehicle to use. Strong communication skills and the ability to build rapport with clients. Ability to follow care plans accurately while being attentive to the individual needs of clients. A background check is required prior to employment. Compassionate nature with a genuine desire to help others improve their lives.
