Job Description
About the Role NYTPS is seeking a Bilingual (Spanish) Early Intervention Ongoing Service Coordinator (OSC) to support families in the Nassau and Suffolk County Early Intervention Programs. We are looking for energetic, organized, and compassionate professionals who are passionate about helping children and families in the community. This is a great opportunity for both experienced professionals and recent graduates looking to grow in the field. Key Responsibilities Guide families through the Early Intervention process, ensuring they understand available services and supports Coordinate evaluations and ongoing services while aligning with family and provider schedules Assist in the development, implementation, and monitoring of the Individualized Family Service Plan (IFSP) Maintain consistent communication with families, providers, and internal staff Monitor service delivery, including: Quarterly progress reports Semi-annual IFSP reviews and updates Accurately document all interactions and case updates in a timely manner Qualifications Candidates must be fluent in Spanish and meet one of the following requirements: Minimum 2 years of service coordination experience , OR 1 year of service coordination experience + 1 year working with infants/toddlers with developmental delays or disabilities, OR 1 year of service coordination experience + an Associate degree in health or human services, OR A Bachelor's degree in health or human services Requirements Must be located on Long Island (Nassau or Suffolk County) Must be able to travel/commute within Long Island for home visits and meetings Strong communication, organizational, and multitasking skills Ability to work effectively in a fast-paced, high-volume environment Reliable transportation required Why Join NYTPS?
