Job Description
Job Summary We are seeking an experienced Qualified Professional (QP) to support 1915(i) services and IDD group-home services. This is a contract, hybrid position, with travel that may be required on a case-by-case basis. The ideal candidate will have experience working with individuals with intellectual and developmental disabilities (I/DD), including experience with 1915(i) services and IDD group-home or residential services. Candidates must meet applicable North Carolina Qualified Professional requirements and have a minimum of 3 years of relevant professional experience. Responsibilities Coordinate and monitor services and supports for individuals with intellectual and developmental disabilities (I/DD). Support person-centered planning and monitor progress toward individual goals and outcomes. Monitor the delivery and quality of services provided. Coordinate with individuals served, families, staff, healthcare providers, and other members of the service team. Provide professional oversight and support related to IDD group-home and residential services. Monitor service documentation, records, and reports for accuracy and completeness. Participate in service planning and team meetings as needed. Monitor the health, safety, and well-being of individuals served. Support compliance with applicable North Carolina requirements, 1915(i) service requirements, and organizational policies and procedures. Provide guidance and support to staff as appropriate. Travel on a case-by-case basis as required. Qualifications Must meet applicable North Carolina Qualified Professional (QP) requirements. At least 3 years of relevant professional experience. Experience working with individuals with intellectual and developmental disabilities (I/DD). Experience with 1915(i) services. Experience with IDD group homes or residential services. Strong communication, organization, documentation, and coordination skills. Ability to work independently in a remote environment. Ability and willingness to travel on a case-by-case basis.
