As a Service Coordinator, you'll help individuals achieve their goals through person-centered planning, advocacy, and coordinated services. What You'll Do Develop and monitor Individual Support Plans in partnership with the people we serve. Advocate for individuals and coordinate services with families, guardians, employers, providers, case managers, and other partners. Coordinate and support behavioral plans and other services. Collaborate with residential teams and direct support staff to promote quality and consistency of care. Maintain accurate and confidential records in myEvolv EHR and other systems. Complete documentation, including progress notes, incident reports, and other required records. Support intake, discharge, authorizations, and follow-up activities. Participate in interdisciplinary team meetings and help monitor progress toward individual goals. Assist with benefits, Social Security, work incentives, and financial supports when needed. Ensure services meet applicable regulatory and compliance requirements, including HIPAA. What You'll Receive Full-Time, Monday-Friday Schedule. Once established in the role, option to work from home 1-2 days per week depending on the schedule to be determined with supervisor Competitive Pay Medical, Dental & Vision Insurance 401(k) Match up to 5%. Two years of experience working with people with disabilities in a service-related field. Strong organizational, communication, problem-solving, and time-management skills. Basic computer skills, including Microsoft Office. Ability to maintain confidentiality and follow HIPAA requirements. Ability to obtain Medication Manager certification within six months . Ability to become proficient in o ur myEvolv EHR and other systems used within six months. Ability to obtain CPR and First Aid certification. Ability to maintain a private phone for work-related communication.
Preferred:
