Responsibilities:
Provide short-term intensive case management for veteran households Develop and monitor housing stability plans tailored to participant needs and goals Conduct comprehensive assessments and maintain regular contact to track progress and service response Assist participants with housing search and placement, benefits enrollment, and access to community supports Complete documentation, reporting, and coordination with service teams to support compliance and service delivery
Qualifications:
At least 3 years of experience in case management, homelessness prevention, rapid rehousing, or transitional supportive services BA in Human Services or a related field Experience in social services, or an equivalent combination of education and experience Demonstrated proficiency with Microsoft Windows, Word, Excel, and Outlook Strong communication and writing skills Ability to work in a hybrid model and in the field
Desired Skills:
Experience working with homeless families or veteran services Familiarity with electronic database systems such as AWARDS Experience in intake, community outreach, or housing-related case management Strong organizational skills and the ability to multitask in a fast-paced environment Military cultural competency or prior service in the U.S. Armed Forces
