Reach-in:

recovery support groups in detention centers for at least 30 days. Provide mental health peer support to facilitate successful transition from incarceration to the community. Identify strengths, abilities and priority needed services to reduce barriers to support successful re-entry into clinical/community based services. Will provide rehabilitative services in both individual and group settings. Assist clients in articulating personal goals for recovery and supports individuals in identifying and creating goals and developing recovery plans with the skills, strengths, supports and resources to aid them in achieving those goals. Assist clients in setting up and sustaining self-help (mutual support) groups, as well as means of locating and joining existing groups. Utilize tools such as Getting into the Driver's Seat of Your Treatment, Preparing for Your Plan, to assist clients in creating their own individual wellness and recovery plans. Assist clients in building skills that will enhance job acquisition and tenure. Teach and role model the value of every individual's recovery experience. Assist client in obtaining affordable housing of his/her choice in the most integrated, independent, and least intrusive or restrictive environment. Models effective coping techniques and self-help strategies. Assist in obtaining services that suit that individual's recovery needs. Assist clients in developing empowerment skills and combating stigma through self-advocacy. Works under moderate supervision with minimal latitude for the use of initiative and independent judgment.
