In this vital role, you will provide essential support to patients by assisting with daily activities, and ensuring comfort and safety. Your dedication will help promote independence, dignity, and well-being for those in need of home and community-based care. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to enhance their comfort and independence. Support patient medication reminders. Provide specialized care for individuals with Alzheimer's disease, dementia, or other memory impairments through tailored activities and attentive supervision. Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems accurately to document patient interactions and medical information securely. Prepare nutritious meals aligned with dietary restrictions; Perform light housekeeping duties such as cleaning patient areas, laundry, and maintaining a safe environment to support overall well-being. Support patients in community settings by helping them attend appointments or participate in social activities to foster engagement. Collaborate effectively with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are followed. Maintain confidentiality of patient information at all times in compliance with HIPAA regulations. Qualifications Proven experience in home health caregiving or long-term care environments such as nursing homes or assisted living facilities. Strong ability to observe patient conditions carefully and communicate findings clearly to healthcare teams. Knowledge of medical terminology and familiarity with EMR/EHR systems for accurate documentation. CPR certfication Preferred; Experience working with individuals who have developmental disabilities or require specialized dementia/memory care is highly valued. Ability to assist with ADLs efficiently while demonstrating patience and compassion. Physical capability to perform transfers using Hoyer lifts or similar equipment; ability to lift or move patients safely is essential. Excellent interpersonal skills for working effectively with patients from diverse backgrounds and their families.
