Job Description
Job Overview We are seeking a compassionate and dedicated person to provide high-quality care to patients in their homes and/or facilities. The ideal candidate will have experience in caregiving, particularly for individuals with developmental disabilities, and will be responsible for implementing care plans tailored to each patient's needs. This role requires a strong commitment to patient care, effective communication skills, and the ability to perform physical tasks such as heavy lifting. Duties Deliver personalized care to patients in their home environment. Assist with daily living activities, including meal preparation and personal hygiene. Implement individualized care plans in collaboration with healthcare professionals. Monitor patients' health status and report any changes or concerns to the appropriate personnel. Provide support and assistance to individuals with developmental disabilities. Perform CPR and other emergency procedures as necessary (If applicable) Maintain a clean and safe environment for patients, ensuring adherence to health and safety regulations. Document patient progress and maintain accurate records of care provided. Qualifications Experience in home care or caregiving settings is preferred. Familiarity with care plans and patient assessment processes. Ability to perform heavy lifting as required by patient needs. Certification in CPR is not required (we will train you if needed). Strong interpersonal skills with the ability to work effectively with patients, families, and healthcare teams. Compassionate nature with a commitment to providing high-quality patient care. Experience working with individuals in group home settings is a plus.
