Duties Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and personal hygiene to promote dignity and independence. Monitor patient health by observing vital signs, reporting changes, and supporting medication administration following care plans and HIPAA regulations. Support mobility and transfers using manual handling techniques such as Hoyer lifts and patient transfer methods to ensure safety for clients and staff. Prepare nutritious meals aligned with dietary needs, assist with feeding when necessary, and maintain a clean and safe environment through regular cleaning duties. Provide companionship and emotional support while engaging clients in activities that enhance their mental well-being, including memory care for individuals with dementia or Alzheimer's disease. Document care activities accurately in client records, communicate effectively with healthcare teams, and adhere to all privacy standards including HIPAA compliance. Assist with transportation to medical appointments or community outings using personal transportation assistance or client transportation services. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients closely, monitor vital signs, and recognize signs of distress or health deterioration. Knowledge of specialized care such as dementia care, Alzheimer's care, neurodevelopmental disorders, autism spectrum disorder support, and intellectual disabilities is advantageous. Certification in First Aid and CPR is required; additional certifications such as medication administration or hospice care are a plus. Experience working with individuals with disabilities or developmental delays is highly valued. Ability to perform heavy lifting safely using proper manual handling techniques; familiarity with patient transfer equipment like Hoyer lifts is beneficial. Strong communication skills for effective client interaction and team collaboration; understanding of home & community care standards is essential.
