Home Caregiver Job Summary:
We are seeking a compassionate and dedicated Home Caregiver to join our team at SYNERGY HomeCare of the West Valley in Phoenix, Arizona. As a Home Caregiver, you will provide high-quality, personalized care to our clients, enabling them to live comfortably and independently in their own homes. - Provide one-on-one care and support to clients in their homes, including assistance with daily living activities, such as bathing, dressing, and grooming
- Assist with medication management, including administration and monitoring
- Prepare healthy meals and snacks, as needed
- Assist with light housekeeping, laundry, and other household tasks
- Promote client independence and autonomy, while also ensuring their safety and well-being
- Develop and maintain positive relationships with clients, their families, and caregivers
- Document all care provided, including medications, treatments, and client progress
- Collaborate with other healthcare professionals to ensure seamless care coordination
Requirements:
- High school diploma or equivalent required
- Ability to lift, bend, and stand for extended periods
- Occasional lifting of 25 pounds or more may be required
- Reliable transportation and valid driver's license (if required)
