Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods and manual handling techniques. Monitor and observe patients' health status by taking vital signs, noting changes in condition, and reporting findings to healthcare professionals. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Administer medications accurately following care plans and ensure compliance with HIPAA regulations to protect client confidentiality. Support clients during medical appointments or community outings by providing personal transportation assistance and ensuring safety throughout activities. Maintain detailed documentation of care provided, patient progress, and any incidents in accordance with home health standards. Implement specialized care protocols for residents with Alzheimer's disease, dementia, autism spectrum disorder, or other developmental disabilities to promote comfort and stability. Requirements Prior experience working with individuals with developmental disabilities, neurodevelopmental disorders, or in assisted living environments is highly preferred. Knowledge of patient observation techniques, medication administration, CPR certification, first aid skills, and vital signs monitoring. Ability to perform heavy lifting when necessary and operate patient transfer equipment such as Hoyer lifts safely. Strong communication skills for effective client interaction and collaboration with healthcare teams while maintaining HIPAA compliance. Experience working within home health or hospice care settings is advantageous. Demonstrated ability to provide compassionate personal care assistance while respecting client dignity and independence.
