Responsibilities Provide comprehensive personal care assistance, including helping with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support Observe and monitor patient conditions regularly, accurately documenting changes in health status using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications safely following prescribed instructions while maintaining detailed medical documentation Support clients with meal preparation, light housekeeping, laundry, and ensuring a clean and safe environment Utilize patient transfer methods safely using equipment like Hoyer lifts or transfer belts to prevent injury for both client and caregiver Communicate effectively with clients about their needs and preferences while respecting patient confidentiality under HIPAA guidelines Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Demonstrated knowledge of home health care assistance and familiarity with home & community care practices Experience working with individuals with developmental disabilities, dementia, Alzheimer's disease or other memory impairments is advantageous Proven ability to handle heavy lifting safely and perform patient transfers using appropriate techniques and equipment Knowledge of medical terminology, medication administration protocols, and hospice or end-of-life care procedures is desirable Strong understanding of home health regulations and compliance standards necessary for delivering quality patient care Previous experience using EMR/EHR systems for documentation and record keeping is beneficial This role offers a rewarding opportunity to provide compassionate care that truly impacts lives.
