Job Overview The Personal Care Attendant provides essential care and assistance with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) to frail elderly participants at our Adult Day Health Center, ensuring safe, compassionate, and high-quality care in accordance with PACE regulations and personalized care plans. Key Responsibilities Direct Care & Support Assist participants with daily living activities (ADLs/IADLs), including bathing, showering, dressing, grooming, toileting, assisted dining, transfers, ambulation, and range of motion activities. Perform safe transfers using Durable Medical Equipment (DME) such as gait belts, Hoyer lifts, and other mechanical lifts. Accompany participants to outside medical appointments and monitor wander-risk participants equipped with code alert devices. Day Center & Operational Maintenance Maintain clean, safe, and orderly center spaces, including kitchens, activity areas, restrooms, and showers. Assist with the food service program by serving meals/snacks, checking food temperatures, reviewing diet orders, washing dishes, and monitoring food expiration dates. Obtain and maintain a Food Handlers Certification within the first few weeks of employment (completed on-site). Communication & Documentation Document provided services accurately and timely within the Electronic Health Record (EHR). Provide timely appointment reminders to participants and respond promptly to complaints or inquiries. Report any changes in a participant's condition to the supervisor immediately. Advocate for participants by reporting any violations of the Participant Bill of Rights & Responsibilities. Maintain HIPAA confidentiality and adhere to all OSHA safety protocols and quality improvement standards. Schedule Monday - Friday, 8:00 AM - 5:00 PM Qualifications Required Minimum of 1 year of experience working with frail, low-income elderly populations. Strong verbal communication skills with the ability to be clearly understood by participants. Solid written communication skills for daily activity charting and documentation. Patient, professional demeanor with a strong customer-service focus for participants, families, and team members. Preferred High School Diploma or equivalent.
