GED CNA Food Handler Certification Full Job Description Job Description:
Reports directly to the PACE Supervisor . Assists participants with personal care and activities of daily living (ADLs/IADLs) . Provides care across multiple settings, including the PACE center, participants' homes, and virtual platforms . Must be able to perform all PCA I responsibilities when assigned. Supports the PCA III by gathering and reporting information related to: Care coordination Participant engagement and participation Maintaining or improving independence and functional abilities Socialization and participant satisfaction Communicates and collaborates with the Interdisciplinary Team (IDT) and other PACE staff to support the participant's plan of care. Monitors and reports participant needs, including: Required supplies or equipment Physical and mental health status Overall well-being and progress toward care goals. Minimum Requirements Certified Nursing Aide education and training. Nurse Assistant Certificate by the State of California, Department of Health and Human Services - Aid and Technician Certification. Current BLS Certificate required. High school diploma required or some college education/credits, preferred. California Food Handler Certificate of Achievement upon employment preferred, must attain w/in 30 days of employment. Prior experience working with physically or mentally impaired older persons strongly preferred.
Bilingual:
English/Spanish/Mandarin/Cantonese depending on location, preferred. Experience working with the elderly patient population.
Skills and Abilities:
Ability to read, write and speak in a clear and professional manner. Ability to complete basic/intermediate math computations. Excellent customer service skills and ability to resolve customer service issues and/or escalate to supervisor as needed. Proficient in beginning/intermediate computer skills. High degree of organization and strong attention to detail. Ability to work well with elderly population. Requires lots of patience and a kind/caring attitude. Ability to work as an effective member of the team. Knowledge of HIPAA Guidelines including the Protected Health Information (PHI) privacy
